Coordination of Benefits
Complete the Coordination of Benefits (COB) and Annual Enrollment Form to update your personal info and add dependents.
Beneficiary Designation
Name your Beneficiary or Beneficiaries for Health & Welfare, Pension & Annuity, and Savings Plan, by completing the only Beneficiary Designation Form.
Direct Deposit
All payments from the Fund Office are electronic. Please complete the online Direct Deposit Form for your Vacation and HRA Benefit payments.
