The Fund has partnered with HCSC to provide supplies (meter, test strips, and lancets) for Diabetes Care at NO COST to the patient. In order to take advantage of this program, please contact HCSC.

All eligible patients treating this condition will be able to have their supplies shipped directly to them from HCSC on a quarterly basis.
To continue coverage all patients will be asked to complete the following preventative health screenings each Plan Year (May 1 – April 30):
- Two registered dietician/nutritionist visits.
- Bi-annual Hemoglobin A1C tests.
- An annual Physical.
- An annual Vision Exam.
- At least 70% compliance for appropriate use of the supplies.
